Overview
Blood Simple is a main quest on Orianna‘s path in Blood and Wine, branching from The Night of Long Fangs. Your choice is only locked in at the very end, when you can still change your mind and go after Syanna instead.
The orphanage
A beast has attacked Orianna’s orphanage and killed nearly all the children. Around the building are bodies, claw marks, footprints and blood, but the key clue is a green mark on a pillar by the entrance, which gives a scent trail. It leads over a ledge to another body, and Geralt concludes the attacker was an alpha garkain. Wagon tracks lead off the road, best followed on Roach, to a wrecked wagon.
The garkain is hiding near a bridge to the northwest. After the fight, a body by the bridge carries a letter from Orianna that reveals something unexpected.
Orianna’s secret
Back at the orphanage, Geralt catches Orianna red-handed: she is a bruxa, and her charity for orphans has a much darker purpose. Geralt lets her go for now, vowing to deal with her one day. She gives him the magic key needed to open the Unseen Elder’s cave.
Birds nearby carry a message to Regis. If you walk to the docks, an alp waits at the city gates. With Regis, you make the final choice: go on to What Lies Unseen, or turn back and find Syanna.
